<p>The YMCA of Austin has a variety of options when it comes to fitness: childcare programs, sports, group exercise classes, swimming, senior programs, dance and more.</p>
<p>Now TexHealth members can enjoy all the benefits of the YMCA through a new partnership with the YMCA of Austin.</p>
<p>When you show your TexHealth member ID to a YMCA staff member upon joining, the YMCA will waive the initiation fee of $48. Additionally, the YMCA will base monthly membership rates on your income, so its services will be affordable to all.</p>

			<content:encoded><![CDATA[<p>This January marks the launch of the TexHealth Central Care Management Program—a health-improvement plan that helps at-risk members achieve their highest level of optimal health.  The program will enable TexHealth members who suffer from chronic health conditions to receive personalized care monitoring free of charge. The program is part of the organization’s broader goal of focusing on early detection and preventive care to keep TexHealth members and the community healthy.</p>
<p>“We are thrilled to finally be able to launch this innovative program that focuses healthcare down to a personal level,” said Dr. Tom McHorse, chair of the TexHealth Board of Directors. “We have planned to implement a care management program, and now we can offer a much-needed service to our members, especially those suffering from diabetes, high blood pressure, and other chronic illnesses.”</p>
<p>The plan will be implemented by Patti Lemon, a registered nurse with more than 25 years experience in the care management and home health industries in Texas. As the Care Management nurse, Lemon will determine personalized medical plans for designated patients that will utilize modern communication technologies and as-needed home visits to provide medication reminders, progress monitoring and counseling. The program will rely on education, communication and accountability to keep members healthy. The ultimate goal is to minimize the risk of hospitalization that patients with chronic health problems face.</p>
<p>“The care management program will be influential in helping TexHealth members experience healthier living,” Lemon said. “It’s uncommon for a health benefits company of this size to be able to offer a customized health service to its members, and I’m delighted to help in the development of this much-needed service.”</p>
