#1: Click on the register now button to the left of the strawberries

#2: Click on the link that says 'Subscriber with no ID card'

#3: Enter your Social Security Number and birthday in the purple identification box

#1: Hover mouse over the employers tab on the top blue navigation bar

#2: Click on the Health Plans link

#3: On the left hand side, click on the Group Coverage link

#4: On the Group Health Plan Search page, roll over the Select a Product box and click United Healthcare Choice Plus

#5: Read through the health plan and call us if you have any questions about UHC coverage
